Attention parents of 6th graders and Seniors!
There are certain vaccine requirements for entering 7th grade and college. In the morning on Wednesday March 26th we will have a vaccination clinic at the school for those required vaccines. Scan the QR code on the flyer or click the link below to sign up:

Please contact Nurse Morgan for questions. Seniors also received a consent packet from the Dept. of Health and those who qualify will receive their vaccines at 10:30 am on Tuesday March 25th. and will attend a presentation about the Texas immunization registry at that time.
Info for 6th Graders:
MCV4 and Tdap (tetanus booster and meningitis vaccines) - both required for 7* grade;
HPV optional
Information for Seniors:
MCV4 - Meningitis vaccine required for colleges
Men B- a 2 shot series for a 5* strain of meningitis that is usually optional, but please check requirements of the college you will be attending

If you have no insurance and will pay out of pocket, please submit your info! You will put "NA" in all insurance blanks.
